support of our powerful national network of 3,500 nurses and social workers and
5,500 discounted providers.
Our eldercare programs offer these outstanding features:
Technology: We have developed web-based applications in conjunction with
professional geriatricians for consumer and professional use. These
applications are integrated with each other and our Customer Contact Center to
offer end-to-end service and reporting. Our consumer web sites,
nursenavigator.com and
livingstrategies.com, include care assessment tools, a reporting tool
and rich content and web links on a wide range of eldercare issues. Our
professional web site includes tools to conduct clinical assessments and track
services performed.
CHCS CareForce Network: Our proprietary national network of over 3,500
nurses and social workers is screened, credentialed and trained to assess,
coordinate and manage the care of an elder loved one.
Customer Contact Center: Our staff of over 100 trained specialists and
over 50 registered nurses provides expert solutions and caregiver support. Full
call-tracking reporting is available for all calls at all levels of detail.
Provider Network: Our network of 5,500 providers across the country
includes nursing homes, adult day care centers, hospices and retirement homes.
We have preferred pricing/access arrangements with these providers based upon
the volume of clients we refer to them. All have been screened, credentialed
and passed state and federal licensing requirements.
Education Programs and Materials: We have developed extensive materials
for training, educating and empowering caregivers. These can be offered at the
worksite or through the web.
