Gary Jacobs
President, CHCS Services, Inc.

Gary Jacobs sets the vision of CHCS Services and develops strategic relationships to advance the company's mission. On a day-to-day basis he is responsible for the company's marketing and sales, public relations, advertising, government affairs and corporate development efforts. Prior to founding CHCS Services, Mr. Jacobs served as the Executive Director of Staff Builders Health Care Services, a Medicare-certified home health agency serving over 5,000 clients weekly. Previously, he was President of HMI, a national HMO management and consulting firm, which he took public in March 1986. Mr. Jacobs received a BA in Political Science from Boston University and a Master's in Public Administration from American University.

Jason J. Israel
Chief Operating Officer, CHCS Services, Inc.

Jason Israel serves as Chief Operating Officer of CHCS Services, Inc. In this capacity, he is responsible for the Company's daily operations. Before joining CHCS Services in 2002, Mr. Israel was President of Bankers Insurance Group's P&C companies in St. Petersburg, Florida. Previous positions include his being a consultant to various insurance companies in operations, strategic planning, disaster recovery and reinsurance. He also served as Executive Vice President of Operations and Administration with American Bankers Insurance Groups in Miami, Florida. Mr. Israel spent 13 years in public accounting. He has a BS in Accounting from Syracuse University and is a CPP and FLMI.

Len Sherman
Chief Financial Officer and Senior Vice President of Finance, CHCS Services, Inc.

Len Sherman has over 25 years of finance and insurance industry experience. Most recently, he served as Senior Vice President of Group and Information Technology for Lafayette Life Insurance Company. He held various executive-level positions at other insurance and financial services organizations, including Allstate Insurance where he worked for 17 years. Mr. Sherman brings considerable experience in leading distribution channels and marketing functions, and is skilled in P&L responsibilities, strategic planning, business development, financial reporting and competitive analysis. He received a Bachelor's degree in Accounting from Indiana University and is a CPA and FLMI. He also holds several NASD licenses.

Felisse Otten, A.R.N.P.
Senior Vice President, CHCS Services, Inc.

Felisse Otten oversees all of CHCS Services' customer contact and policyholder service areas and also manages CHCS Services' Eldercare Services division. She was President of WorldNet Services Corp. prior to its consolidation with CHCS Services. While at WorldNet, she also held positions as Quality Assurance/Special Projects Director and Vice President of Operations. Prior to working at WorldNet, Ms. Otten held administrative positions with the South Broward Hospital District and Mount Sinai Medical Center (South Florida). Ms. Otten is a licensed Advanced Registered Nurse Practitioner with more than 20 years of clinical experience. She is a graduate of the University of Miami and holds a Master's of Science from Florida International University.

Maureen Lillis, RN, CCM
Senior Vice President, Long Term Care Division, CHCS Services, Inc.

Maureen Lillis has over 25 years of clinical experience working as a professional nurse specializing in geriatrics. Ms. Lillis has worked extensively in the clinical areas of long term care service delivery in both nursing homes and home health agencies. During her more than 12 years in the insurance field, Ms. Lillis also worked at MetLife and at Celtic Life Insurance. Ms. Lillis received her Associate degree in nursing from the University of Kentucky. She is a Certified Case Manager specializing in long term care insurance management.

James Kalmer
Chief Underwriter, CHCS Services, Inc.

James Kalmer provides underwriting and claims expertise to develop appropriate strategies and procedures to achieve bottom-line mortality and morbidity objectives. He also trains CHCS Services personnel in the underwriting and claims areas. The first 31 years of Mr. Kalmer's career were spent at The Equitable Life Insurance Company in New York where he held various management positions, including Senior Vice President and Chief Underwriting Officer. In 1996 he joined PennCorp Financial as Vice President and Chief Underwriting Officer, where he provided underwriting and claims oversight for the family of companies that made up PennCorp Financial. Mr. Kalmer is a graduate of Saint Vincent College with a Bachelor's degree in economics and he holds an MBA from New York University.
