Eldercare Support & Assistance
Because care issues can arise any time, CHCS Services Eldercare Service Center provides geriatric support & assistance for the member or their support system. Our multi-lingual Senior Care Advisors provide information and can talk your members through any eldercare issue.
Information and Referral Services:
CHCS Services provides caregivers with telephonic consultation for eldercare concerns through a group of trusted professionals that they can turn to when faced with the complicated issues associated with finding the best solution for their elder.
These professionals are experienced in answering common eldercare questions; assisting with complex problem solving; helping members make informed decisions about care options and assisting families with locating nursing homes, assisted living facilities, home health care options, retirement communities and adult day care centers anywhere in the nation. Our specialists will also identify and provide referrals to other local community based resources that may be appropriate, including but not limited to, senior centers, transportation providers, and “meals on wheels”.
Access to Nationwide Provider Network:
Elders and caregivers will have access to providers including home health care agencies, assisted living facilities, nursing homes and durable medical equipment companies. All providers have been screened, credentialed and passed all state and federal licensure requirements. The network is continually growing and expanding to bring in the best quality providers, at the best rates for our clients.
Telephonic Wellness Assessment (1 per year):
Each year, CHCS Services will conduct a telephonic Wellness Assessment of the elder covering six (6) domains: health, nutrition, psychosocial status, memory, functional abilities and personal safety. After this assessment, a detailed telephonic report is provided for the member and/or family member if designated.
Access to Professional Care Management Services:
In-depth, face-to-face expert assistance is provided through our nationwide network of credentialed and trained professionals, the CHCS Services CareForce™ network. Our CareForce™ network consists of nurses, social workers and healthcare professionals with training and experience in eldercare issues. Our CareForce™ network uses our proprietary technology platform with quality and consistent service delivery. Our professionals help caregivers meet the challenges of eldercare by assisting families with answers on nearly every senior issue from simple suggestions for healthy living to detailed plans of care.

CHCS’ web-based service offers the “self-help” approach to planning for, learning about, or actively managing aging-related issues 24 hours a day, 365 days a year. At our program website elders and caregivers have access to our specialized self-assessment tools, knowledge building resources and practical tips to help manage common aging issues and the complex responsibilities of care giving. By using these interactive tools, member caregivers can better understand the elder’s situation and his or her own situation as a caregiver.
